Self Management UK provide support and advice for people living with a long term health condition, such as asthma, diabetes, heart disease, COPD, arthritis and chronic pain.

Most people living with a long term condition are managing day-to-day life with that condition by themselves. Self Management UK are the market leader in self management, working with both patients and those caring for someone living with a condition to help minimize the impact the condition has on their lives. By providing the right tools, skills and patient-centered support, they give people the confidence to take a proactive role in managing their condition and therefore improve their overall wellbeing.


Both generic and disease-specific programmes are offered, often accessible at local community venues or online. They also work with specific groups such as carers.

Self Management for Life Programme

Generic or tailored programme consisting of 7 weekly sessions lasting 3 hours each. The programme addresses issues from diet, lifestyle choices and medication usage to communication skills, relaxation tools and increasing self-confidence.
