The specialist palliative care service aims to improve the quality of life of patients and their families facing problems associated with life threatening illness through the prevention of and relief of suffering.
Nurses see patients on the ward and provide advice and support to the patient and their family. This may be in the form of treatment plans, information and planning relating to their future care. They can also give advice on controlling symptoms should this be required. The nurses work closely with the Palliative Care Consultants and will ask them to be involved with the care if necessary.
Referrals to the service can be made by doctors, nurses and all professionals on the wards.
Families and carers can request the involvement of the palliative care team at any time via any health professional.
The service is availabile Monday-Friday, 09:00-17:00.
