Sandwell Dementia Navigators is a service for people living with Dementia and their families, carers and supporters. During the coronavirus crisis, the Dementia Navigators will provide a telephone advice and support service for people living with dementia and their carers. This includes providing advice on how to live safely whilst following the guidance from the Government and the NHS on precautionary measures to prevent the spread of COVID-19.

The Dementia Navigators can also help with shopping for food and essential supplies, delivering prescriptions to people’s homes and signposting to helpful organisations in the community. The Dementia Navigator service is accepting new referrals by telephone – 0121 553 6483 choose option 6 – and by downloading and completing the Sandwell Dementia Navigators referral form. You can email the team at:  dementianavigators@sandwellcrossroads.org

The Dementia Navigator service is a partnership between Agewell, Sandwell Crossroads and Dementia Pathfinders.
