Helping primary care to support people with dementia

Sandwell Dementia Navigators

October 29 2020

Sandwell Dementia Navigators, a partnership between Crossroads, Agewell and Dementia Pathfinders, is for people living with dementia and their families.

The service is designed to reach people in communities and care homes living with dementia with the aim of improving access to diagnosis and post-diagnosis support, equipping family carers with information, knowledge and skills and promoting positive community responses to enable people with dementia and their families and supporters to live well and with meaning and purpose.

Coronavirus support

During the coronavirus crisis, the Dementia Navigators will provide a telephone advice and support service for people living with dementia and their carers. This includes providing advice on how to live safely whilst following the guidance from the Government and NHS on precautionary measures to prevent the spread of Covid-19.

The Dementia Navigators can also help with shopping for food and essential supplies, delivering prescriptions to people’s homes and signposting to helpful organisations in the community.

The Dementia Navigator service is accepting new referrals by telephone – 0121 553 6483 (choose option 6) or please download and complete the referral Form.
