Helping primary care to support people with dementia

Dementia Friendly Practices

Dr Katrina Webster West Hampshire CCG Clinical Director for Mental Health, Jason Hope Senior Commissioning Manager and Wessex Deanery Quality Improvement Fellow Dr Verity Turner have been working with three surgeries as they explored the process of becoming ‘dementia friendly’.

The project identified many resources that can support this process. The practices started with the principles of the iSPACE Dementia Friendly GP Surgeries work and resulted in resources that the project and surgeries used in the process.

These include:

Signing up to the Dementia Action Alliance to identify an action plan for the surgery and suggested actions; DAA blank Action Plan ; Suggested DFP Action Plans

A ‘cultural web’ tool designed to look at how the day-to-day practices within a surgery influence the environment for patients with dementia, their carers and the staff. Cultural Web Tool

Patient and carer leaflets


Much of the work has been identifying existing resources and encouraging the sharing of these. The Dementia Roadmap is a platform for this to continue.
